I hereby acknowledge and agree that in the event of a medical emergency or deterioration in my dog's health during the program, Pack Life LA holds the right to seek immediate veterinary care from a licensed veterinarian or emergency clinic. I understand and accept that Pack Life LA and its staff are not liable for any charges incurred for emergency veterinary care, and I am solely responsible for all associated costs, including examination fees, diagnostic tests, treatments, medications, and surgical procedures. While Pack Life LA will make reasonable efforts to contact me or my designated emergency contact in case of a medical emergency, I authorize them to proceed with necessary medical care without delay if immediate action is required for the health and safety of my dog, even if I cannot be reached for consent. I also acknowledge my responsibility to verify and maintain adequate pet insurance coverage for my dog, recognizing that insurance can assist in offsetting the financial burdens associated with emergency veterinary care. Finally, I confirm that I have provided Pack Life LA with all relevant information about my dog's health history, including pre-existing medical conditions, allergies, or medications, and I authorize the sharing of this information with the attending veterinarian in the event of an emergency. *

If the animal’s health demands quick action, Pack Life LA is authorized to have a veterinarian look at and treat the animal for the cause and effect. Such expenses, being reasonable in amount, shall be paid promptly by the owner. *

I assume all liability for any harm or damage caused by my pet(s) to Pack Life LA, property, or other animals during the boarding period. I release the Caretaker from any liability related to the boarding, except in cases of gross negligence or intentional misconduct. *
