This waiver is necessary for full registration and participation in the Pack Life LA Group Class. Please complete it in full and submit it at least 48 hours prior to your scheduled class (when possible).

Assumption of Risk

I understand that participation in dog training classes or workshops involves potential risks, including but not limited to injury to myself, my dog, or others. I voluntarily assume all risks associated with participation.

Health & Vaccination

I confirm that my dog is in good health, is free from contagious diseases, and is current on all required vaccinations.

Liability Release

I agree to release and hold harmless Pack Life LA LLC, its employees, trainers, volunteers, and affiliates from any and all liability, claims, damages, or expenses arising out of participation in any training activity.

Responsibility for Damages

I accept full responsibility for any damage or injury caused by my dog to property, people, or other animals during participation.

Travel

Additionally, I acknowledge that I am solely responsible for arranging and managing my own transportation to and from training locations, including any off-site travel required for the Group Class.

Photography/Videography

I grant Pack Life LA permission to use photographs and/or videos of me and my dog taken during the workshop for promotional and educational purposes.
